One – two palliative patients are without a family physician in the community per month, estimates Home Care.
Home Care is asking PCNs to help build a list of family physicians who would be willing to take on these unattached or orphaned palliative patients when needed so their team can focus on new consults.
What to know:
- The request is to identify family physicians willing to take on these patients to facilitate prescribing, symptom management, etc.
- Triplicate prescribing is required.
- Home visits are a plus but not required; the nurse case manager can be the “eyes” in the home and video visits can be facilitated as needed.
- Support from the Palliative & End-of-Life Care (PEOLC) consult service would be available to you.
