Home and Community Care services realignment

To better meet the growing demand, Home and Community Care services in the Calgary area have recently been internally realigned to:

• Allow more people to receive the care they need in their own homes and communities
• Focus on chronic disease management versus disease-specific care

Changes: The previously specialized Heart Failure, COPD, and Dementia Care home care teams are transitioning into geographically based Home and Community Care teams.

People will continue to receive care through their Home and Community Care teams, and specialized support and consultation will remain available as needed.

Current resource levels will be maintained.

You can continue to refer patients as normal through Community Care Access.
