Contact usOur Social Worker team helps patients and families navigate the health system and connect with community resources supporting their health and well-being.
Referrals can be made for patients of all ages who are experiencing concerns that affect their:
- Quality of life
- Living conditions
- Ability to access health care
- Ability to manage the effects of illness, complex health concerns, or life stressors
At the initial appointment, a Social Worker meets with the patient to assess needs and identify resources. Home visit assessments may take longer.
If services or supports are identified, the Social Worker reviews options with the patient. Together, they develop a plan that may include referrals to community organizations, follow-up appointments, or check-ins.
Appointments can take place at the Primary Care Centre, by phone or video, or in the patient’s home, depending on their needs. Social Workers meet with patients as often as needed to help them access ongoing assistance.
Our Social Workers provide short-term support and are not intended to replace long-term case management. For patients with more complex needs, the Social Worker may remain involved while helping establish longer-term community support.
Information patients share with a Social Worker is confidential. A consult report outlining the agreed-upon plan will be sent to the referring physician. Written consent is required before the Social Worker exchanges information with other agencies, except when a safety concern must be reported.
Patients must be attached to a CWC PCN physician member. Referrals can be submitted using the CWC Primary Care Centre referral form.
