Our Mental Health Program helps patients understand their support needs and select a care pathway that works for them.
Pediatricians, physicians, and nurse practitioners with Standard, Enhanced, or Comprehensive membership/service access can refer patients to the program’s centralized mental health referral service for a phone consultation with a Primary Care Registered Psychologist (PCRP).
Who can be referred?
The program supports patients who are attached to a CWC PCN physician or nurse practitioner and receive ongoing care through that practice. This keeps mental health support connected to the patient’s medical home.
How to refer
Complete the referral form, available on your EMR and cwcpcndocs.com. Whenever possible, complete the form with the patient so they understand the reason for the referral and what to expect.
If the form is not available in your EMR, download it from cwcpcndocs.com and upload it to your EMR files. Clinics that do not use an EMR can fax the printed, completed form to 855.966.4102.
In an emergency, advise the patient to go to the nearest emergency department, call 911, or contact the Distress Centre at 403.266.HELP (4357).
What patients can expect
Within seven business days of referral, a Patient Care Coordinator will call to schedule a PCRP consultation. During the consultation, the PCRP works with the patient to understand their concerns, identify their goals, and discuss their options.
Referrals may address concerns such as:
- Substance use
- Anxiety
- Depression
- Grief and loss
- Health concerns
- Life transitions
- Personal growth
- Relationships
- Trauma
A full list of referral reasons is included on the referral form.
The PCRP may use a questionnaire to assess the patient’s distress. All patients receive self-management resources after their consultation. Those ready for more support may be referred to community mental health services, such as Community Connect YYC or the Calgary Counselling Centre.
