From September to November 2024, we administered our annual Membership Check-in Survey with our Enhanced and Comprehensive members to gather feedback on our Patient’s Medical Home (PMH) programs and teams. The survey included two sections:
- A self-assessment tool to promote reflection on progress in building a strong PMH
- A Health Team Effectiveness (HTE) tool to assess the strength and functioning of primary care teams
Survey highlights:
- We saw an increase in PMH self-assessment scores, with physicians averaging a score of 93 out of 124 (a six-point increase from 2023). The domains with the largest improvement were “Engaged Leadership”, “Quality Improvement”, and “Panel and Continuity”.
- Strong HTE scores were reported, with an average of 4.3 out of 5, suggesting that primary care team members generally view their team as effective.
- The highest scoring area of team effectiveness was “Acting and Feeling Like a Team” (4.44).
These scores are used to evaluate our services for areas of strength and opportunities for improvement.
We are encouraged by these positive results and will continue to engage with our members to enhance our team-based supports.
