To refer, please complete the referral form in your EMR — search for “CWC PCN 24h BP Referral Form” — (or use the PDF referral form) and fax it to the location of your choice.

All the providers on the 24-hour BP monitoring form provide:

• 24-hour blood pressure monitoring with no cost for your patients
• A report back to you with interpretation of the results
• The ability to book urgent referrals when indicated
