Celebrating 20 years: A look back at our journey as a PCN

Marking a major milestone: Two decades of support, innovation, and collaboration in primary care.

 

After 20 years, we reflect on everyone whose contributions led our PCN to this point with gratitude:

  • Our founders, the five physicians who first envisioned our PCN.
  • All the physicians who have been part of our membership and engaged with our programs and services for their patients and with us as an organization.
  • All our team members who built our PCN into what we are today and helped us support our members and community.

Our PCN came into being in August 2006 with about 60 physician members — the first of many significant milestones for our organization.

To celebrate and reflect on our journey, we invite you to explore a digital timeline that highlights our development, key milestones, and contributions throughout the years.

We look forward to continuing to evolve, reaching more milestones, and supporting members and their patients now and in the future.

The timeline tells our story across five chapters as we developed our programs and services for you and evolved as a PCN:

  • Our early years (2006-10) with the very beginning of our PCN, our first PCN-run clinics and health professionals, and the start of our support for older adults.
  • Building on our foundation (2011-16) as we revamped our after-hours program, started teams of health professionals, and added a new coordinator role to support screening.
  • Enhancing our work (2017-20) with more support for older adults, a dedicated team for members’ questions and input, and a new membership model to provide sustainable services and more fully meet the new objectives for PCNs — plus a special section on the pandemic.
  • Our latest chapter (2021-26) when we started our Mental Health Program and our programs for physiotherapy and nutrition counselling, plus enhanced screening projects.
  • An evolving organization (2026) with our overall support for members and patients, efforts toward inclusivity, and how we work to improve with data, resources, and evaluation.
