As we head into the spring months, we look forward to celebrating National Nursing Week from Monday, May 11, to Sunday, May 17.
This is an excellent opportunity to reflect on the work our nurses do to support patients as they navigate various health conditions.
Our Primary Care Registered Nurses (PCRNs) use a patient-centred approach to support chronic disease management. They can provide information, support, and assessments for conditions including:
- Smoking cessation
- Weight management
- Diabetes
- Heart health
- And more
Through collaboration and education, PCRNs help empower patients to make meaningful lifestyle changes that can improve their health and well-being.
As part of our new inter-team referral process, PCRNs can now directly refer patients to our Social Workers, Mental Health Program, Dietitian Program, Physiotherapy Program, and Access Appointment Service. This process will further improve our PCRNs’ ability to connect patients with key resources and streamline their care experience.
We greatly value our nurses and the support they provide. This value was further highlighted by recent member feedback, which showed our nurses enhanced patient care by providing follow-up appointments, ongoing condition management, and detailed disease education.
