Palliative care request for family physicians

Jun 16, 2025

One – two palliative patients are without a family physician in the community per month, estimates Home Care.

Home Care is asking PCNs to help build a list of family physicians who would be willing to take on these unattached or orphaned palliative patients when needed so their team can focus on new consults.

What to know:

  • The request is to identify family physicians willing to take on these patients to facilitate prescribing, symptom management, etc.
  • Triplicate prescribing is required.
  • Home visits are a plus but not required; the nurse case manager can be the “eyes” in the home and video visits can be facilitated as needed.
  • Support from the Palliative & End-of-Life Care (PEOLC) consult service would be available to you.

If you have questions, please contact [email protected].

If you are interested in joining this list, please contact your Physician Liaison. Home Care’s palliative team would then reach out based on region when needed.
