You know older adults are more likely to have multiple health issues and need additional time and expertise.

So, to support you and your older patients, our specialized Senior Services team:

• Includes registered nurses and physicians, a psychiatrist, and a pharmacist with geriatric expertise
• Can complete in-depth assessments that may include cognitive screening, environmental risk assessment, fall assessment, functional assessment, medication review, and nutritional assessment
• Sends you a consultation note after each encounter

They can also play a key role as advocates for patients who may be isolated and assist patients and families with navigating the healthcare and community support systems.

Senior Services can see patients in their home and/or at the Primary Care Centre.

All members can refer their patients aged 65 and up or physiologically aged to Senior Services:

• Please provide a detailed referral on the referral form and complete its safety section in case of a home visit.
• Our team will connect your patient with the appropriate team member or combination of team members based on patient needs.
• We encourage referrals to be made before patient conditions become critical.

Most commonly, your patient’s first appointment will be an in-home assessment by a registered nurse with the program. (Less commonly, the first appointment could be a dual in-home visit by the registered nurse and a member of our social work team or an in-clinic appointment with the full Senior Services team.)

After the in-home assessment, the registered nurse could then continue to follow the patient as appropriate or connect them to the full, multidisciplinary Senior Services team (nurse, physician, psychiatrist, and pharmacist) or other resources.

The number of visits and team members involved depends on the patient’s needs.
