Diabetes management is one of the top reasons for referral to our Primary Care Registered Nurses (PCRNs). In 2024:
- 23 per cent of PCRN appointments focused on diabetes concerns
- Eight per cent of appointments focused on pre-diabetes concerns
Understanding this condition affects a significant portion of our population, our nurses use a patient-centred approach to help in every phase of your patient’s diagnosis: From preventative care to disease management.
Pre-diabetes, preventative support could include:
- Education and support with dietary recommendations and life changes
- Behaviour change support
- Blood sugar management support
Support for newly diagnosed patients could include:
- A thorough review of pathophysiology
- Blood glucose monitoring
- Education on the signs and symptoms of hypo and hyperglycemia, with examples of treatment options
- Insulin titration and management education
- A review of recent labs, with additional testing recommendations if needed
- Introducing a schedule for diabetes-related illness screenings
For patients living with diabetes, support could include:
- Assessment and appropriate care for diabetes distress
- Foot assessment and appropriate foot care
- Pharmaceutical education and navigation
Education and resources for self-management are provided at all stages to increase patient confidence and help them become active participants in self-managing their condition. Patients may also receive help with connecting to community resources, managing mental health, and setting goals.
