On the heels of our 20th-Anniversary Report, we have updated cwcpnreports.com with our latest annual report for a focused look back on our 2025-26 fiscal year.

Each year, our annual report highlights the many initiatives and accomplishments of our PCN across our four priority areas for our members, teams, and the public — a chance to catch up on our progress.

• Expand the eligibility criteria for referrals to our Dietitian Program and Physiotherapy Program to help more patients
• Engage with members about our teams and improve our data collection and use
• Support our growing membership with educational opportunities, communications, and assistance with meeting the Physician Practice Improvement Program requirements
