Get one-on-one dietitian support for your patients with PCOS so they can live healthier lives
Offering nutrition counselling is recommended for all patients with polycystic ovary syndrome (PCOS) by the provincial primary care clinical pathway.
And, after carefully monitoring capacity, we are expanding eligibility for our Dietitian Program to include patients with PCOS — an underserved need for nutrition counselling identified by our physician members.
All members can refer their patients to our Dietitian Program if they are aged 10 and up and:
- Have one or more eligible primary concerns (PCOS, IBS, Crohn’s disease, ulcerative colitis, celiac disease, liver health, or kidney health)
- Do not have coverage by other health payers or the financial means to access a dietitian
To refer:
- You can immediately download the updated referral form, upload it to your EMR, or print it to make a referral.
- To save time, our Health Information Coordinators will add it to clinic EMRs, where possible, and members can contact their Physician Liaison if they need additional information to add the form.
- Please note the added relevant labs requested (only if already available) for PCOS on the second page of the updated form to assist our dietitians.
We will continue to monitor and evaluate our program as part of our goal to provide you and your patients with the most helpful services and resources.
Dietitian Program refresher
Eligible, referred patients receive a one-hour, one-on-one appointment with one of our Primary Care Registered Dietitians to expertly assess their needs and determine the most appropriate type of care.
Appointments are primarily virtual with an in-person option.
Patients are then connected to the most appropriate type of care:
- Continued one-on-one appointment(s) with our Primary Care Registered Dietitian team for nutritional counselling within the program (30-minute follow-up appointments)
- Referred to an AHS, community-based, or Primary Care Alberta service or provided self-management resources
- Referred to a Primary Care Registered Nurse (for patients of physicians with Enhanced or Comprehensive membership)
